    Ghana – 13th September, 2017: A 10-year-old boy has died after he took a 75 milligram of Naklofen drug administered to him at the Tema General Hospital, mother of the deceased claimed.

    Mrs Mabel Senahey said the doctor had prescribed 50mg of the drug for her son, but the pharmacist changed it.

    New Mexico, US – 5th September, 2017:  A couple has filed a lawsuit against San Juan Regional Medical Center and a Farmington medical clinic citing medical negligence in the delivery of their daughter that caused severe and debilitating injuries following her birth.
    Adam and Jolinda Morrissey filed the lawsuit against the hospital and Northwest New Mexico Women's Health Specialists, LLC on July 20 in district court, according to court records.

    Levenshulme, UK – 29th September, 2017: A mother-of-two died of a heart attack after going to her GP surgery 13 times in a year to seek help for chest pains, which she was told was stomach ache.

    Michelle Ditcher, 33, had a well-documented family history of relatives having cardiac failure at a young age, but an administrative error kept these hidden from her doctors.
