The patient with chronic heart disease and history of strokes, was admitted at the hospital. An elective surgery was performed for pacemaker implantation. She was on drugs to manage her heart condition, which was discontinued for twenty-four hours in view of the surgery.

The patient suffered a stroke after the surgery, which was swiftly managed by the attending cardiologist and other doctors. She was shifted to ICU, the drugs to control her heart ailment were restarted. The patient was subsequently discharged after a couple of days.

The patient experienced pain and stiffness in her left breast. She consulted the local doctor, but to no avail. She then consulted the doctor at a maternity centre. The doctor clinically diagnosed breast cancer; thereafter performed excisional biopsy to confirm the diagnosis.

About a fortnight later, the patient followed up with excruciating pain around her waist. She was referred to an orthopaedic specialist. But to no avail.

The patient, a young boy and the only child to his parents, was admitted at the hospital with complains of body ache and high fever. He remained hospitalised for four days during which the doctors conducted several tests and adopted a conservative line of treatment.

On the third evening, the boy’s urine output had reduced and he also developed respiratory problems. The young lad was referred to another hospital, from where he was referred to another higher centre for better medical care. Unfortunately, he died the same evening at this third hospital. The cause of death was septicaemia with disseminated intravascular coagulopathy with acute renal failure. Malaria was the root cause of boy’s death.

The patient, a pregnant woman, experienced severe pain in her abdomen. She consulted the doctor at a hospital. USG was performed; it reported stones in gall bladder.

The patient was admitted for surgery. Spinal anaesthesia was administered by the anaesthetist, the surgeon performed cholecystectomy to remove gall stones. After about forty minutes, the patient suffered cardiac arrest. While resuscitation was successful, she slipped into a coma.

A middle-aged woman consulted the surgeon with complains of numbness, weakness, slurred speech, severe headache and loss of vision. USG was performed; it reported stones in common bile duct (CBD).

The surgeon performed surgery to remove the stones. While the intra-operative period was uneventful, the post-operative period was fraught with complications.

The patient, suffering from pain in her knees for about six months, consulted doctor at a reputed private hospital. She was diagnosed with advanced osteoarthritis. The doctor advised Bilateral Total Kneel Replacement (BTKR).

She heeded to the advice, got admitted at the hospital. Pre-operative tests, including ECHO, were performed. The ECHO test reported 35% LVEF, hence, patient and her family were concerned about BTKR.

There is a need for defining protocols for showing the implants to patient / attendants before implanting them. The doctor and hospital in this case did not, which was an innocent mistake, but they were held negligent for it.

The patient was admitted at the hospital with complaints of severe left ventricular dysfunction with a history of repeated hospitalizations due to lower respiratory tract infection (LRTI), congestive heart failure and pedal edema. She received decongestive therapy with dobutamine infusion and intravenous diuretics which improved her condition.
