The patient was admitted to the hospital with complains of fever and other complications. He was diagnosed with multi-organ dysfunction syndrome; ARDS, septicaemia and malaria were also suspected.

Several tests, including chest X-ray were performed, but definitive diagnosis could be made. He was put on ventilator to manage respiratory distress. A month later, he was discharged in a stable condition.

The patient experienced stomach pain hence, she consulted the doctor at hospital. Ultrasound was performed; it reported cholecystitis.

The doctor performed laparoscopic cholecystectomy, assisted by anaesthetist, a team of doctors and para-medical staff. Admittedly, anaesthesia could be administered in the second attempt.

Post-operatively, the patient developed right foot drop. Despite consultation and treatment by several doctors, including patient’s preferred doctors, there was no improvement in the condition.

The importance of taking patient’s history and noting it in medical records has a huge importance – from a legal stand point and in delivering quality healthcare. This case is one of the best examples of the fact.

The patient experienced severe pain in both lower limbs. He consulted the doctor who diagnosed grade III weakness in the right foot. An MRI report brought by the patient reported prolapsed disc at L3, L4, L5 and S1. He also suffered from urinary problems.

The patient experienced chest pain and anxiety early in the morning. He was rushed to the hospital by his daughter. The doctor on duty diagnosed acute coronary syndrome and admitted the patient to ICU where he underwent thrombolysis.

Unfortunately, the patient suffered a fatal cardiorespiratory arrest and died few hours later.

The patient, a seventy-four-year-old woman, suffered brain stroke. She experienced breathing difficulties and felt drowsy; hence, she was admitted to the hospital in ICU.

She suffered another stroke that led to aspiration of her stomach contents in the lung. Critical care was provided, including ventilation. Unfortunately, bed sores developed which aggravated her condition.

Patient’s husband sought DAMA and took her home. However, her condition was irreversible; she died after few days.

The patient experienced vision problems. He consulted the eye surgeon who performed cataract surgery. An innocuous and fairly common procedure turned out to be quite a nightmare for the patient.

He visited the doctor very next day with complain of severe pain in the operated right eye. The doctor changed the bandage, prescribed medicine and drops, and advised to follow up on the third day.

Condition of patient’s right eye deteriorated fast. Despite following up several times, there was no improvement. Fed up with the complications, the patient consulted couple of other eye specialists. To his utter shock, both doctors diagnosed endophthalmitis. Eventually, the infected right eye had to be operated. The patient lost complete vision in the right eye.
