The patient was taken to hospital as he was experiencing chest pain. He was initially treated conservatively.

ECG was performed, chest X-ray was taken; and after some time Trop-T test was performed. The Trop-T test confirmed acute myocardial infraction (AMI). For some reason, the treating doctor and cardiologist did not admit the patient immediately.

About two hours later, patient started gasping for breath and suffered a stroke and died.

A middle-aged man had persistent vision problems. Initially, he sought medical care from an eye centre, but did not get the desired result. Therefore, he approached an eye specialist, who diagnosed Retinitis Pigmentosa (RP) and cataract in both eyes.

Scleroplasty was performed, patient was discharged a couple of days later. Unfortunately, he suffered post-operative complications and nearly lost vision in both eyes. He sued the doctor.

The patient suffered from persistent vomiting and loose motion. He was initially treated at a nursing home, but to no avail. Subsequently, he was taken to a hospital where doctors diagnosed kidney problem.

A central venous catheter was inserted, after which blood clot developed. The patient suffered paralysis stroke.

He was referred to a higher centre, but was brought back to the hospital. The patient received treatment and physiotherapy at the hospital for about three months before being discharged.

A pregnant lady was receiving antenatal care at the hospital. She gave birth to a premature, but a healthy baby girl. After discharge, she experienced excruciating abdominal pain and bouts of vomiting. The patient was readmitted.

Unable to diagnose the exact cause of pain, the hospital referred patient to another hospital. Here too, the doctors could not pinpoint the cause of pain. The patient was then taken to a third hospital, and again to yet another hospital.

The patient was diagnosed with gall stones. Laparoscopy was performed by the surgeon at hospital. During the procedure, it was discovered that some stones were stuck in the liver, hence, an open surgery had to be performed.

The patient suffered post-operative complications. He was admitted in the ICU, blood had to be transfused, but unfortunately, he slipped into coma. Eventually the patient was referred to a higher centre.

The patient consulted doctor for fertility issues. Chocolate cyst in the right ovary was diagnosed. The doctor performed diagnostic scopy and oophorectomy, the cyst and right ovary were removed. However, consent was obtained only for diagnostic scopy and removal of cyst.

Post-operatively, the patient continued to experience severe abdominal pain. CT scan reported ‘both ovaries appear normal’. The patient was prescribed medicines, but to no avail.

She consulted doctor at another hospital. CT scan reported ‘right ovary not visualized, with presence of a foreign body’. Surgery was performed to remove the foreign body, which turned out to be a mop pad.

The patient was diagnosed with kidney stones. Surgery was performed, DJ stenting was done. The surgeon advised him to report after six weeks to remove DJ stent. The patient was beneficiary of State Government medical care benefit scheme.

The patient visited for follow up as advised, but did not want to spend for the procedure to remove DJ Stent. He followed up after about nine months for removal of DJ Stent.
