A 53-year-old woman underwent surgery for a right ovarian cyst at a nursing home. She had a history of previous abdominal surgeries, making adhesions and altered anatomy relevant risks during the procedure.

Following surgery, she developed persistent fever, vomiting and abdominal complications. She was subsequently readmitted and later shifted to another hospital, where a ureteric injury was identified and further surgery was performed. Her condition eventually deteriorated, with sepsis and other complications, and she died.
