Inquiry ordered into medical negligence at CHC Kellar

The authorities today ordered an inquiry into the alleged medical negligence that led to the death of a pregnant woman at Community Health Centre (CHC) Keller in south Kashmir’s Shopian district.
The Directorate of Health Services, Kashmir (DHSK) stated that an inquiry committee has been constituted to ascertain the circumstances that led to the patient’s death.
“Taking cognizance of the unfortunate incident involving the death of a pregnant woman at CHC Keller, Shopian, the Directorate of Health Services, Kashmir, has constituted an inquiry committee to conduct an in-depth investigation into the circumstances and causes leading to the patient’s demise,” DHSK said.
It added that the committee has been tasked with evaluating all aspects of the case, including medical care provided, adherence to clinical protocols, and any lapses or deficiencies in the healthcare delivery process.
“The inquiry will be carried out in a time-bound manner, and appropriate action will be taken based on the findings and recommendations of the committee,” the statement said.
The deceased, 27-year-old Shazia Waseem, wife of Waseem Ahmad Lone from Chawan Keller, reportedly died due to medical negligence.
The Chief Medical Officer (CMO) Shopian informed that a three-member committee has been constituted to investigate the circumstances leading to her death and determine the exact cause.
According to the CMO’s order, the patient was admitted to CHC Keller on October 8, 2025, and reportedly passed away during the night between October 8 and 9.
The committee will also identify any possible negligence, lapses, or systemic failures on the part of the hospital staff and recommend corrective measures, including administrative or disciplinary actions if required.
